Initial Visit

We want to make your initial visit to Dr. Gallagher's as smooth and easy as possible.  Here are two forms every patient is required to complete for their initial visit.  We have supplied them here for you print out and complete in at home.  By bringing the completed form with you, we can start you initial visit in a timely manner.

During your initial visit, we will also take digital X-rays, which are less radioactive than normal X-rays, and document the current state of you teeth, gums, tongue, and overall mouth condition.  After accessing your current condition, we will schedule you for a follow up to perform any needed services.
